3. This form is to be used to make a formal complaint with the City of North Miami. All complaints will be assigned for investigation within 24 hours of receipt of the complaint. Please print all of the information requested below, and mail or hand-deliver the completed form to the Code Compliance Unit office. Attach any photos, and/or other evidence to this form.
5. New Florida Law Prohibits Anonymous Code Enforcement Complaints

   A new Florida state law, Senate Bill 60, prohibits local governments from investigating anonymous code complaints effective July 1, 2021. This new legislation prohibits Code Officers from initiating investigations of potential violations of codes and ordinances by way of anonymous complaints. It also requires persons reporting potential violations to provide a name, address and phone number upon filing the complaint.
   If the violation presents an imminent threat to public health, safety or welfare or imminent destruction of habitat or sensitive resources, then the prohibition does not apply.
