FORM DS-DE 9 Appointment of Campaign Treasurer and Designation of Campaign Depository.
Before opening the Campaign Account to accept contributions or making expenditures, this form must be on file with the Qualifying Officer.
The Campaign Depository must be a bank, savings and loan association or credit union authorized to do business in the State of Florida. The campaign account must be separate from any personal or business accounts.
All monetary contributions must be deposited in the campaign account within five (5) business days of receipt. Only the campaign treasurer or the deputy campaign treasurer may sign campaign expenditure checks.
Each candidate must file a statement with the qualifying officer within 10 days after the Appointment of Campaign Treasurer and Designation of Campaign Depository is filed. Willful failure to file this form is a first degree misdemeanor and a civil violation of the Campaign Financing Act which may result in a fine of up to $1,000, (ss. 106.19(1)(c), 106.265(1), Florida Statutes).
